This report is required for ANY Accident, Incident or Injury occurring on FHPC courts or surrounding property
IF A LIFE THREATING INJURY OR FATALITY OCCURS,
CALL 911 IMMEDIATELY & NOTIFY ANY FHPC BOARD MEMBER
Please report all incidents by completing this form, then placing it in the box on the outside of the West equipment box within 48 hours of an occurrence.
